My Personal Health Record (PHR)
Personal Health Record (PHR) is your secure online account with Devon Sexual Health. It makes booking appointments and managing your sexual health simple and confidential.
With your PHR, you can:
- Order postal STI testing kits
- Book and manage your own sexual health clinics appointments online
To use these services, you’ll need to set up a PHR account. It’s quick, simple, and keeps your information safe.
With your PHR, you can’t:
- Book appointments with your GP
You can find your nearest GP or sexual health clinic using our find a service tool
What You’ll Need
- Your own email address (not shared with anyone else)
- Your own UK mobile number
How to Create Your Account

- Go to the sign-up page: Register for a PHR
- Verify your email and phone number – we’ll send you codes.
- Add your details:
- Full name
- Date of birth
- Home address
- Gender identity
- Sex assigned at birth
If you are already registered, you can log into your account here.
Your Privacy Matters
Your information is private and secure. It is not linked to your main NHS record or NHS number.

To create your Personal Health Record (PHR) account, you’ll need:
- Your own email address (not shared with anyone else)
- Your own UK mobile phone number
Please make sure:
- You do not use a shared email address
- No one else has access to your phone
Using a Shared Device?
If you have to use a shared computer or phone, follow these tips to keep your information safe:
- Don’t save your login details
- Don’t leave the screen unattended
- Turn off any features that store passwords
- Clear your browsing history when you’re done
If these steps aren’t followed, there’s a risk your private information could be seen by someone else. Accounts created with someone else’s email or phone will be deactivated until we can verify your identity.
By Registering, You Agree:
- We can contact you using the email and phone number you provide
- Your details are accurate, up to date, and only accessible by you
- Your email and phone accounts are password-protected
Sometimes we may need to verify your identity remotely via email.
Please note: The PHR system does not allow shared email addresses. You must register with an email that is personal to you.
If you need any of the services listed below, don’t use the online booking system. These services are time-sensitive.
Emergency Contraception
The sooner you use emergency contraception, the more effective it is.
- Use it if:
- You had unprotected sex
- A condom broke or slipped
- You forgot your pill
- Or had sex without any contraception
Emergency contraception is available from:
- Sexual health clinics
- Some GP surgeries
- Pharmacies – no appointment needed, no prescription needed
You can search for emergency contraception services near you on the NHS website.
PEP (post-exposure prophylaxis)
If you think you’ve been exposed to HIV (for example, through sex with someone who may have untreated HIV) PEP is available from
- Sexual health clinics
- Hospital emergency departments
To be effective, it needs to be taken no later than 72 hours (3 days) after exposure – the sooner the better. It’s not a replacement for safe sex or PrEP, and only partially prevents HIV.

Rape or Sexual Assault
If you’ve been raped or sexually assaulted, contact the Devon and Cornwall Sexual Assault Referral Centres (SARCs) on 0300 303 4626.
SARCs provide medical, practical, and emotional support with specially trained NHS staff.
